Role of social worker - challenges and limits of work

1 Abstract The diploma thesis "The role of a social worker in a psychiatric hospital - challenges and limits of work" deals with the profession of a social worker in a psychiatric hospital. The diploma thesis normally consists of a theoretical and an empirical part. The theoretical part is focused on mental illness, the characteristics of serious mental illness, the profession of a social worker working in a psychiatric hospital, multidisciplinary cooperation, the specifics of working with the mentally ill, the reform of psychiatric care and the organization of inpatient psychiatric care in the Czech Republic. In the empirical part, the qualitative research methodology is described and the data, which are the result of semi-structured interviews with social workers and their clients, are interpreted. At the end of the thesis, the main research question is answered, recommendations for practice and proposals for further research are included. Involving users in the psychiatric care reform

The subject of this diploma thesis is the involvement of users of psychiatric care in the process of planning and realization of psychiatric care reform at the macro level of policy making. The aim of the research is to describe and understand this involvement. In the research are describer possibilities of participation in the phases of involvement, its form, purpose, formal regulation of opportunities, expectations of respondents, perceived power and barriers in involvement. The data was collected during interviews with key actors and studies of relevant documents. The results relate to the theoretical concepts of coproduction, patient-oriented care, strategic and human rights documents (the Convention on the Human Rights of People with Disabilities). The element of power is described through the typology of Sherry Arnstein's participation ladder. Evidence of interviews indicates elements of tokenism and disadvantages. There is a low (one-person) representation of users in reform bodies, lack of time for discussion in consultative bodies, lack of information and lack of capacity (people, time, resources) for greater development of patient organizations. The Issue of Availability of Psychotherapy Through the Lens of Key Stakeholders.

The bachelor thesis focuses on the issue of availability of psychotherapy in the Czech Republic. The work analyzes the possible causes of unavailability of psychological services for their users and interprets the future of the system of providing mental health care with special emphasis on psychotherapy. To answer the research questions, semi-structured interviews were conducted with actors with different perspectives on the issue. The interviews provided not only a description and evaluation of the current and future state of the mental health care system, but also suggestions for its improvement. The conceptual and theoretical framework of the work consists of a model of actors generating the agenda in arenas (also known as the A-A- A model) and policy analysis. As the main factor influencing the availability of psychotherapy, respondents mentioned the generally poor systemic setting of its provision. However, respondents agreed that the Czech Republic is well placed to improve the overall system of mental health care through the ongoing Psychiatric Care Reform, which should respond to the shortcomings of the system through the strategy document Psychiatric Care Reform Strategy (2013). Issue of mental health care in the Czech Republic with a focus on the personal experience of the users

In my diploma thesis I would like to introduce the readers to the issue of mental health care in the Czech Republic. I will focus on mental health definition, classification of mental illnesses, and also introduce services provided to people with mental illness. Furthermore, I will deal with the systém of mental health care in the Czech Republic, where I will try to point out the greatest positives and also some negatives of this system. The reader will also learn about Czech legislation related to this issue. Finally, I will inform about the psychiatric care reform, which started in 2013 in the Czech Republic - I will mention the main milestones, plan and strategy of this reform, and the benefits of this reform will be demonstrated on particular steps and realized projects. In the research section, I will focus on personal experience of care users, where thanks to the interviews, I can mediate the biggest mental health care problems from users point of view. The system and the financing system of psychiatric care in the Czech Republic in the period 2010-2016

The bachelor thesis focuses on mental health, which is becoming a socio-economic problem these days. Since there is a constant prevalence of patients with mental illnesses and thus an increase of a burden in the economic system, this work explains the system of psychiatric care, which is crucial for the future development of mental health of Czech population. Based on the analysis of the issue it is evident that the effort to manage the care system to community care will do not contribute to the reduction of treatment costs, but on the other hand, will facilitate the accessibility to psychiatric facilities for patients. The funding of psychiatric care is not based on the national health system, which influences the direction of a large part of the funds for treatment in psychiatric hospitals.
The motivation of general nurses for work in a psychiatric hospital

Motivation is a force which modifies and influences human behaviour in working process. It´s very important for nurses to be motivated at work education and the chance of its use in nursing profession represents one of the significant motivators. The aim of this work was to elaborate the basic motivational and demotivational factors for nurses used when deciding for the job in a psychiatric hospital and explore if qualifying education knowledge and study practice affect the decision for the job in a psychiatric hospital. Regarding these targets, the following hypotheses were set: H1: Meaningfulness of the work represents the main motivational factor for the profession choice in a psychiatric hospital. H2: Fear of the potential threat from the patient is the main demotivational factor. H3: Knowledge from the qualifying education doesn´t affect the job choice in a psychiatric hospital. H4: Practice during the studies has motivational impact on the following choice of work in a psychiatric hospital. To obtain the necessary data, the quantitative research was used with a questionnaire as a research instrument. There were 100 nurses working in psychiatric nursing included in the study, who received qualifying education at the higher medical school as gualified general nurses or at the university in nursing bachelor studies. Based on the results obtained, we examined that the interest in psychiatric nursing is thought to be the primary motivational component, but not the meaningfulness of the work. Psychical intensity of the work was classified as the main demotivational factor, not the fear of the potential threat from the patient. Regarding the education and practice it was noted, that the knowledge from the qualifying education doesn´t affect the job choice in a psychiatric hospital and practice during education has motivational impact on the following choice of work in a psychiatric hospital. As an outcome of the research the recommendation was created, which can be used as a scheme for qualifying education in terms of motivation and effective preparations of nurses for their work.
